Forms families bring us

  • Travel consent forms for a child flying with one parent, a relative, or a group
  • School, camp, and athletic authorization forms that require a notarized signature
  • Medical consent forms naming a caregiver while parents are away
  • Other guardianship and permission forms that call for a notary

Simple, up-front pricing

Every appointment is a little different, so we talk pricing with you directly — no confusing rate sheets. Reach out by phone or email, or just book, and you'll have a clear quote before anything is scheduled. No hidden fees, no surprises.

How it works

  1. Have the form ready

Fill out your form completely before the appointment — we notarize the signature, so the wording and content are up to you or the organization requesting it.

  2. Meet in person or online

We come to you anywhere in the service area, or handle it over secure video. The signing parent or guardian just needs a valid government-issued photo ID.

  3. Signed, sealed, ready to go

We verify identity, witness the signature, and apply the seal — then your form is ready to hand in. Witnesses can be arranged if your form requires them.

A quick note before you book

We notarize the signature on your form — we can't advise on which form you need or how to word it. The school, airline, or organization requesting it can tell you exactly what they require; then we'll make the signing easy.

Minor Consent questions

Can you notarize a travel consent form for a minor?

Yes. Travel consent forms are one of the most common documents we handle for families. Have the form filled out and ready, and the signing parent or guardian present with a valid photo ID.

Do both parents need to be there?

It depends on what your form and the receiving party require. Whoever is signing needs to be present with valid ID at the time of notarization. If your form calls for witnesses, tell us when you book and we'll arrange them.

What other consent forms can you notarize?

School, medical, athletic, and other authorization forms for minors. We notarize the signatures — for questions about which form you need or how to complete it, check with the organization requesting it.
